The UK patient information leaflet says each Mounjaro KwikPen contains four doses. It acknowledges that a small amount of medicine may remain after those doses have been given correctly, then says: “Do not try to use any remaining medicine.”
What does “golden dose” mean?
“Golden dose” or “fifth dose” is wording used in online Mounjaro discussions for liquid that can still be seen in a KwikPen after its four labelled doses have been used. It is not the name of an authorised dose, a separate medicine or an instruction in the UK product information.
That distinction matters. A forum nickname can make the remainder sound like a benefit built into the pen. The official wording describes it only as medicine that may remain and should not be used.
What does the UK patient leaflet say?
The current UK patient information leaflet on the electronic Medicines Compendium gives three relevant instructions together:
“Each KwikPen contains 4 doses of Mounjaro… A small amount of medicine may remain in the pen after all doses have been correctly given. Do not try to use any remaining medicine. After administration of the last dose, the pen must be properly discarded.”
This answers the practical question without requiring a calculation: the labelled pen contains four doses, the leaflet anticipates a visible remainder, and it says not to use that remainder. PCG does not provide extraction methods, click counts or instructions for turning it into another dose.
What does the product information confirm?
The UK Summary of Product Characteristics (SmPC) is written for healthcare professionals. It confirms that the pre-filled KwikPen is a multiple-dose pen containing four doses of 0.6 ml that can be administered. It then says: “Any excess solution in the pen after use should be discarded.”
The SmPC also says the leaflet and the pen instructions must be followed carefully. Neither source presents the excess solution as a fifth labelled dose. Neither gives a method for extracting it.
What the official sources do not say
The patient leaflet does not promise a particular amount of leftover liquid. It does not label the remainder as a spare dose, give it a strength or say that it can be measured accurately. The official documents therefore do not support treating whatever remains in an individual pen as a standard extra dose.
Online posts may show people using syringes, counting clicks or making their own calculations. Those are not instructions from the UK leaflet or SmPC. This page deliberately does not reproduce them.
What does the leaflet say about disposal?
The patient leaflet says the pen must be properly discarded after the last dose. It also says not to throw medicines away through wastewater or household waste and to ask a pharmacist how to dispose of medicines no longer used. The SmPC says medicinal waste should be disposed of in line with local requirements.
A provider should be able to explain the local disposal route that applies to the pen and any needles supplied with it. If that information is missing, ask the provider or dispensing pharmacy rather than relying on a general online instruction.
What should a UK clinic make clear?
The leftover-liquid question is a useful test of whether a provider gives device-specific support rather than leaving patients to forums. Before paying, check whether the clinic or pharmacy makes these points clear:
- the KwikPen is labelled as a four-dose pen;
- the leaflet says not to use medicine remaining after those doses;
- patients can contact a named clinical or pharmacy team with device questions; and
- the provider explains how used pens and needles should be disposed of locally.
This is not a judgement about an individual pen or personal treatment. If the pen appears damaged, does not work as expected or the instructions are unclear, use the contact route supplied by the prescriber, clinic or dispensing pharmacy.
